Please complete the following items and return to the Dean’s Office in the College of Science, Health, and the Environment. Alternatively, you may also email the documents to cshe@mnstate.edu. All items must be submitted to be considered for a committee interview and committee letter of recommendation.

  • Current resume
  • Personal statement
  • MCAT scores OR National Standardized Examination Scores (MCAT, DAT, etc.)
  • Transcripts (provided by Dean’s Office)
